Task Comment Editor Fixes and Security Improvements

We've fixed issues with saving list content in comments and improved document access controls.
What was happening
- When you added bullet or numbered lists in a task comment, parts of the list could disappear after you saved.
- Long comments could overflow the comment area, making the editor awkward to use.
- Keyboard shortcuts in the comment editor were inconsistent, so sending a comment from the keyboard didn’t always work.
- In some cases, documents from entities you don’t have access to could still appear in Dashboard and Archive views.
What we fixed
- Lists (including nested lists) in task comments save correctly and keep their formatting.
- The comment editor stays within its panel and scrolls properly for long comments.
- Keyboard behavior is reliable: Ctrl + Enter sends your comment, and Tab / Shift+Tab indents or outdents list items.
- Document visibility now consistently respects access permissions, so you only see documents for entities you’re allowed to view.
